非酒精性脂肪肝(NAFLD)是成人最常见的肝病类型,其患病率在世界各国均在上升。该病病理特点是肝脏脂肪堆积,在不饮酒、无病毒感染或其他肝脏疾病的情况下肝脂肪超过5%的肝脏重量。自从NAFLD与肥胖、胰岛素抵抗、糖尿病和血脂改变联系在一起,它被认为是代谢综合征的肝脏表型。NAFLD的发病机制尚未明确,但不同的诱因例如脂质蓄积、胰岛素抵抗、氧化和内质网应激、线粒体功能障碍和炎症均可引发。改变生活方式是构成NAFLD控制的第一要素。营养干预包括低脂和具有较高不饱和脂肪酸的碳水化合物饮食。此外,补充抗氧化和细胞保护剂对减少氧化应激、炎症和纤维化也有效。体育锻炼能减少脂肪合成基因的表达、脂肪堆积或胰岛素抵抗并且提高心肺功能。有氧运动和阻力训练的益处已被发现,甚至在运动停止后也存在。然而,对于营养和运动干预的更多分子和细胞机制任需要更多研究来分析,并确定所需的运动量和其与体重减轻的联系。在这篇文章中,我们提供了涉及NAFLD进展机制的最新综述,并分析了营养干预和体育锻练在这种情况下的预防和治疗益处。

摘要: Non-alcoholic fatty liver disease (NAFLD) is the most common liver disease in adults and its prevalence is rising around the world. This pathology is characterized by accumulation of liver fat, which exceeds 5% of liver weight in absence of alcohol consumption, viral infection or other hepatic etiology. Since NAFLD has been associated with obesity, insulin resistance, diabetes or alteration of lipid profiles, it is considered as the liver manifestation of metabolic syndrome. Pathogenic mechanisms of NAFLD have not been clearly elucidated, but different events such as lipid accumulation, insulin resistance, oxidative and endoplasmic reticulum stress, mitochondrial dysfunction and inflammation are involved. Modifications in lifestyle constitute the first line for the management of NAFLD. Nutritional interventions include low fat and carbohydrate diet with higher polyunsaturated fatty acids ingestion. Moreover, supplementation with antioxidant and cytoprotective agents could be useful to decrease oxidative stress, inflammation and fibrosis. Physical activity enables to reduce the expression of lipogenic genes, fat accumulation, or insulin resistance and improves cardiorespiratory fitness. Benefits have been found following both aerobic exercise and resistance training, and remain even after exercise cessation. However, more studies are required to analyze the molecular and cellular mechanisms involved in nutritional and physical intervention, and to define the volume of activity required and its association with weight loss. In this paper, we offer an updated overview of the mechanisms implicated in the progression of NAFLD, and analyze the beneficial effects of nutritional interventions and physical exercise in the prevention and treatment of this condition.
